Traumatic
Brain Injury Waiver
The
Traumatic Brain
Injury (TBI) Waiver provides
funding for home and community-based services
for children and adults who have an acquired or
traumatic brain injury; TBI Waiver services may
be provided in a person' own home, the home of
their biological or adoptive family, the home
of a relative, in either a family or corporate
foster care home, a board and lodging facility
or in an assisted living facility; if the person
is married, they may receive TBI Waiver services
while living with their spouse.
ProWorks
provides prevocational services
through the TBI Waiver; these services are designed
to prepare an individual for paid or unpaid employment
as noted in their care plan.
Eligibility
for TBI Waiver services is determined through
a screening process; to be eligible, a person
must: |
.
be a Medical Assistance recipient or be
eligible for Medical Assistance based upon
their own income or assets; |
.
be certified "disabled" by the State Medical
Review Team or the Social Security Administration;

.
be under the age of 65 years when the waiver
is opened; |
.
have a diagnosis of traumatic or acquired
brain injury or an acquired or degenerative
disease diagnosis where cognitive
impairment is present, provided the
diagnosis is not congenital; |
.
experience significant/severe behavioral
and cognitive problems related to
the injury; |
.
be determined to need the level of care
provided in a nursing facility or neurobehavioral
hospital; |
.
be assessed at Level IV or above on the
Rancho Los Amigos Levels of Cognitive Functioning
Scale, and |
.
choose care and services in the community
instead of a nursing facility or neurobehavioral
hospital. |

Once
eligibility is determined, the following considerations
must also be taken into account: |
.
Are the services necessary to ensure the recipient's
health, welfare and safety? |
.
Is the service covered by another funding source,
for example, Medical Assistance state plan services,
private health care coverage, Medicare, education
or Vocational Rehabilitation Service funding?

Have all options been assessed, and does this
option meet the individual desires, needs and
preferences of the person? |
.
Is the cost of the service reasonable and customary?
|
